US-Integrated Chronic Care Pilot
Pharm to Table
Through our work across the globe, we have been inspired by how the concept of health is integrated locally through food, social connections, movement, and community. What we have observed is that health means more than the quality of the system of care, the resources, and the access. It embodies the place one lives, their community, their feeling of safety, and overall well-being.
In the face of the increasing burden of non-communicable diseases globally, it is now even more important to understand how lifestyle impacts health. This means that integrating chronic care includes understanding the lifestyle, the people, the food system, the environment, the community, and the local health system and working to collaborate and adapt care to prevent, treat, and reverse these conditions. In this effort, the Alliance is excited to partner with the WellFed Community and Artistree gardens to pilot a program in Tampa, FL on integrating chronic care management and lifestyle medicine through learning about sustainable food systems, incorporating the concept of food as truly medicine in the teaching kitchen, discussing health, and growing the community from the Pharm(Rx) to the Table.
